Who we are

NursingHomeLens is operated by MarkAlarm LLC, a Florida limited liability company, doing business as NursingHomeLens (“we”, “us”). Payments are processed by Stripe; charges appear on your statement as NURSINGHOMELENS. Support: support@nursinghomelens.com.

What the service is — and is not

We provide analysis, comparison, and plain-English translation of official records published by the Centers for Medicare & Medicaid Services (CMS). We are not affiliated with, endorsed by, or sponsored by CMS, Medicare, or any government agency. We provide no medical, legal, or placement advice; we do not recommend, rank, or match facilities; and nothing on this site or in a report is a substitute for your own judgment or professional advice. Star ratings shown are CMS's Five-Star ratings, not ours.

Citations and findings are official records of what state inspectors reported on specific dates. Facilities may dispute findings, and a record is not by itself a conclusion about conditions at a facility today. We correct verified errors within 72 hours of a report to corrections@nursinghomelens.com.

The Facility X-Ray report ($49, one-time)

The report is a snapshot of CMS data as of the date stamped on it, for the facilities you selected, delivered as a PDF at a permanent link and by email. If something is wrong with your report or you're not satisfied, contact support within 30 days of purchase. Refunds are limited to one per customer, and a refund ends your access to the purchased report — refund and continued access are alternatives, not both.

Acceptable use, liability, and changes

You may share a purchased report privately with family and advisors. You may not republish reports or bulk-scrape this site commercially. The service is provided “as is”; to the maximum extent permitted by law, our total liability for any claim relating to the service is limited to the amount you paid us in the twelve months before the claim. We may update these terms; material changes will be dated at the top of this page. These terms are governed by the laws of Florida.
